Forms
New patients
New patients must call to make their first time appointment. An online intake form will be sent to the email you provide when you create an account or schedule your first appointment. You will not need to print out the form and can fill it out from your phone, tablet or computer. Please fill out your online form before your appointment. If you’re unable to fill it out in advance, please arrive at least 15 minutes early to fill it out at the clinic.
If the online form is not accessible for you, paper copies are also available at the front desk if needed.
Missed or cancelled appointments with less than 12 hour notice incur a $20 fee.
Appointments last about an hour, with first time visits sometimes lasting a little longer.
